- Completed and signed application by the applicant (Form 1).
- Photocopy of the entire passport or another equivalent document (see table No. 4).
- Civil status documentation confirming the family relationship.
- Photocopy of the income tax return of the foreign family member with whom family cohesion is being requested.
- Certificate of housing suitability according to Article 29, paragraph 3, letter a) of Legislative Decree No. 286/98 and subsequent amendments related to the family composition.
