- Application completed and signed by the interested party (Form 1);
- Copy of the entire passport or another equivalent document (see table no. 4);
- Regarding the requested update, the following documentation must be provided:
- Change of address: It is sufficient to complete Form 1 as a self-certification.
- Inclusion of a minor child born in Italy: It is sufficient to complete Form 1 as a self-certification.
- Inclusion of a minor child who entered Italy following the issuance of an entry visa for family reunification or family members accompanying: It is sufficient to complete Form 1 as a self-certification, attaching a photocopy of the passport or equivalent document with which the minor entered Italy.
- Update of passport or equivalent document data: It is sufficient to complete Form 1, attaching a photocopy of the new passport or equivalent document.
- Registry changes: For changes related to name, surname, date and place of birth, and citizenship, a declaration issued by the Diplomatic/Consular Representation of the country of origin must be provided, attesting that the new personal details refer to the same person.
- Update of photographs: If the residence permit is used as a personal identification document, the photographs must be updated every five (5) years; it is sufficient to complete Form 1.
