- Completed and signed application by the applicant (Form 1).
- Photocopy of the entire passport or another equivalent document (see table No. 4).
- Declaration from the responsible person of the religious community in Italy, attesting to the nature of the position held, responsibility for food and lodging, and stamped by the Bishop’s Curia or equivalent religious authority in Italy.
- Photocopy of the insurance policy, valid within the national territory for the entire duration of the requested residence permit, covering the risk of illness and accidents.
NOTES:
The documents mentioned in items a, b, c, and d are required for both the initial residence permit application and the renewal application.
