- Completed and signed application by the applicant (Form 1).
- Photocopy of the entire passport or another equivalent document (see Table No. 4).
- Photocopy of the custody decree.
NOTES:
– Exempt from stamp duty.
– The application is signed by the guardian until the fourteenth year of age of the ward, by both if the age is between fourteen and eighteen years.
